What Is Mechanical Low Back Pain?
Mechanical, or non-specific, low back pain describes pain that comes from the joints, muscles, ligaments, and discs of the spine working together, without one single structure being clearly identifiable as the cause on examination or imaging.
It is by far the most common category of lower back pain. The term reflects the reality that pain is often influenced by how the spine is loaded and moved rather than by damage to any one structure.
This is different from a specific diagnosis such as a disc herniation, spinal arthritis, or sacroiliac joint dysfunction, where a particular structure has been identified as the primary driver.
Mechanical low back pain is typically aggravated by certain positions or movements and eased by others. It also tends to fluctuate with activity levels, posture, and stress.
When a More Specific Pattern Is Present

Symptoms
Symptoms commonly vary with movement, posture, activity, and time spent in one position.
- A dull, aching, or sometimes sharp pain across the lower back that may vary from day to day
- Pain that is worse with prolonged sitting or standing, bending, or lifting
- Symptoms that improve with position changes and movement
- Stiffness after rest or a sense that the back needs to loosen up before it feels normal
- Usually no significant burning or shooting pain travelling down the leg, unlike sciatica
What the Research Shows
Research supports spinal manipulation and manual therapy, especially when hands-on care is paired with active rehabilitation.
- A Cochrane review by Rubinstein et al. (2011) found that spinal manipulative therapy produced meaningful reductions in pain and improvements in function for people with chronic low back pain. Effects were broadly similar to other evidence-based treatments and better than sham or ineffective therapies.
- A randomised trial by Ferreira et al. (2007) compared general exercise, motor-control exercise, and spinal manipulative therapy. Manipulation produced slightly better short-term functional improvement than general exercise and results similar to motor-control exercise, although the three approaches evened out over the longer term.
- An umbrella review by Conde-Vázquez et al. (2025) reviewed 21 systematic reviews covering more than 35,000 participants and found that manual therapy, including spinal manipulation and soft-tissue techniques, produced meaningful short-term improvements in pain and disability for chronic non-specific low back pain.
- Broader guidance from the Lancet low back pain series by Hartvigsen et al. (2018) and Foster et al. (2018) supports pairing hands-on care with an active management plan for the best long-term outcomes.
