Tarsal Tunnel Syndrome - Ankle Pain

Back of ankle pain occurs due to entrapment of the posterior tibial nerve within the tarsal tunnel. The tarsal tunnel is a narrow space that lies on the inside of your ankle. This is similar to a familiar wrist syndrome, called carpal tunnel syndrome.

Symptoms affected the inside of the ankle or the bottom of the foot and present as;

  • Numbness/tingling
  • Weakness
  • Aggravated by activity, relieved by rest

This occurs as we have a thin tissue, called the flexor retinaculum, which holds muscles, nerves and ligaments in place. These structures that pass through the flexor retinaculum include; posterior tibialis muscle, flexor digitorum longus muscle, tibial artery, tibial vein, tibial nerve and the flexor hallucis longus muscle. When there is pressure or tension in the area, it causes the tibial nerve to become compressed. This is why the numbness, tingling and weakness occurs.

This can occur from injury, repetitive use injury, compression, callus formation, inflamed or enlarged tendon sheath or be associated with osteoarthritis, rheumatoid arthritis or post trauma ankle deformities.

These symptoms are exacerbated during weight bearing activities and relieved with rest. Patients can report nocturnal symptoms, which can be eliminated with elevation of the foot.

Depending on which of the 6 structures are affected, you can have numerous problems. The flexor digitorum longus muscle is responsible for bending of the toes. Flexor hallucis longus flexes your big toe. Tibialis posterior and flexor hallucis longus perform pointing of the foot. Digitorum longus muscle helps to twist the foot. Therefore, if you are having any difficulties with this movements, let one of our chiropractors know!
